Membership Application

We are a Private Club, and all prospective Members are required to complete our Membership Application.  While seemingly inconvenient and intrusive, we ask for this information for the safety, protection, and comfort of our existing Members.  If you have any questions or reservations about our procedures or the information that we gather, please do not hesitate to contact us to address your questions/concerns before you complete our Membership Application.

All information we collect from you will be kept strictly confidential and secure.  We will never disseminate, sell, distribute, or otherwise share any information that you provide to us.  All information collected is used strictly for the purpose of screening you for Membership and for administrative purposes once you become a Member of our Club.

We will verify the information that you give us, but we will do so anonymously, and we will in no way compromise your privacy or in any way disclose any of the information that you have provided or even the fact that you have applied for Membership with us.

If you are uncomfortable with providing us with the requested information, perhaps our Club is not a good match for you.  All of our existing Members have entrusted us with the requested information and have been subject to the same screening and verification of that information - it is our duty to insure that all prospective Members are subject to the same level of scrutiny.  Our Members have told us unequivocally that they want the comfort of knowing that the person sitting next to them has been screened and approved for Membership before being allowed to join our Club.

Please provide your REAL name or reasonable abbreviation or derivation thereof.  What is on your Driver's License, or what does your mother call you?  We don't need your nickname here - that will come later.

Ditto.  This MUST match the name on your Driver's License or other State-issued identification.  If you are approved for Membership, you must bring your Driver's License or other valid, government-issued photo ID on your first visit.
   
Nickname
What do you go by?  What would you like us to call you (within reason)?  No, we're not going to call you "Pimp Daddy", "Spartacus", "Long Dong Silver", "Sugar Britches", etc.  If you don't have a nickname, don't worry.  We're certain that our friendly, courteous, professional staff will be able to create one for you.
   
E-mail address  *
We will use this e-mail address for correspondence with you regarding your Membership Application.  If you are approved for membership, we will also use this address to send you regular news and updates on our Club.  You may unsubscribe from our e-mail list at any time.
   
Date of Birth (MM/DD/YYYY) *
We use this information to differentiate you from other Members with the same or similar names.  For instance, if your last name is Jackson, Johnson, Smith, Lee, Nguyen, or anything else that is fairly common, then we will need your DOB to differentiate you from other members with the same or similar names.  This must also match the DOB on your Driver's License or other State-issued identification.  ALL Members MUST be at least 21 years old.  If you are approved for Membership, you must bring your Driver's License or other valid, government-issued photo ID on your first visit.
   
Contact Telephone Number (preferably Cell #)  *
We will only call you if necessary to complete the processing of your Membership Application.  Subsequently, we will not call you without your permission, and we will never send you text messages without your permission.
   
Where Do You Work?  *
We will NEVER attempt to contact you at work.  We ask for your employment information ONLY for one-time verification of your employment.  All verification will be done anonymously, and we will NEVER disclose who we are or the fact that you have applied for Membership in our Club.  If you are approved for membership, on your first visit you must bring an ID Badge, payroll check stub, or other documentation verifying the employment information that you provided.
